PHARMACY POLICIES
1. Prescription Policies:
a. All prescriptions are filled with generic drugs. The pharmacy will not honor prescriptions that are marked "do not substitute" as the availability of a name-brand product can not be guaranteed.
b. Prescriptions must be written legibly in ink or typewritten. The prescriber's signature MUST be written in ink--stamped signature prescriptions will not be honored. If the prescriber's name and phone number are not pre-printed on the prescription blank, it must be legibly printed on the prescription form. The patient's name and the date must also be written on each prescription. Any changes made on the prescription should be corrected by crossing-out the error and re-writing the information. In all cases, any changes made to a prescription must be initialed by the prescriber.
c. Controlled substances must be written on separate prescriptions from other medications. The physician's DEA number must be on all prescriptions for controlled substances. The pharmacy does not keep a log of physician's DEA numbers. The quantity to be dispensed must be written numerically as well as spelled-out as a word (i.e. # 30, thirty). Schedule II medications must be filled within seven days of the date written. The pharmacy will not fill prescriptions for controlled substances written by prescribers for themselves or their family members.
d. Prescriptions from civilian practitioners for eligible beneficiaries will be honored provided the prescribed medication is routinely stocked. We strongly suggest that all medications be written on separate prescriptions to prevent inconvenience if a medication is not stocked or cannot be filled at our pharmacy.
e. Due to workload and staffing constraints, the pharmacy cannot accept new prescriptions or refill authorizations via telephone or fax.
f. Quantities of maintenance medications are limited to the quantity written on the prescription, a maximum of a 90 day supply. Please limit "PRN" medications to a quantity sufficient for an episode of care and add refills as necessary. Quantities of controlled substances in Schedules III-IV may not exceed a 60 days supply and Schedule II 30 days supply (except methylphenidate and dextroamphetamine when ADD or ADHD is specified a 60 day supply is authorized). Pharmacists will adjust refills on maintenance medications to conform with the dispensing limitations. Refills will be honored for no longer than one year from the original date of the prescription for non-controlled medications and six months for refillable controlled drugs.
g. Although medications are provided to patients free of charge, this service is not free--the American taxpayer pays the bill. Please consider less expensive alternatives wherever possible when prescribing.
